While the entire Martini family is on holiday in Puglia, a mysterious man leaves an envelope addressed to Elena, Lele's deceased first wife, in the mailbox of the villa in Poggio Fiorito.
Valerio, Elena's former piano teacher, tries to get to know Anna with the excuse of a rap concert and seems too interested in the affairs of the Martini house. Meanwhile, Sara, without consulting Lorenzo, signs the compromise for the purchase of a villa.
Lorenzo learns that the tendon in his hand is torn and blames Sara for what happened. The woman, mortified, decides to take a walk on the beach. There, by chance, she comes across a party where, between one beer and another, she meets Andrea. Completely drunk, the two end up in bed together.
Magdalene is summoned to the Vatican for a private audience with Pope Francis, but Elena and Bobò have a hand in the summons letter. Anna, meanwhile, continues her preparation for the admission tests to Medicine, but her meetings with Valerio and Geko are increasingly frequent.
Anna, accompanied by her grandparents and the lucky bear of the Martini family, takes the entrance exam for the Faculty of Medicine. Meanwhile, Lorenzo decides to admit Celeste, a young resident, into the operating room, but the girl panics and runs away.
Libero and Enrica invite Valerio to dinner to repay him. For this reason, Lele is forced to reveal the secret that binds him to the man. In the meantime, Lorenzo hires Andrea to renovate the new house, when Sara meets him she remembers the wild night she had with him.
Libero, still shaken by the revelation about Valerio, tries to attack the man, ordering him to stay away from Anna. Meanwhile Emiliano, at the suggestion of Ginevra, his new girlfriend, goes to Lele's clinic to give him the money he owes to the young Martini in order to end all relations with the past.
It's Anna's birthday. To celebrate, the young Martini gathers relatives and friends at her home, including Valerio and Geko. At the end of the evening Lele confesses the truth about Valerio and the paternity test to Libero, Enrica, Sara, Lorenzo and Maddalena.
Lele is determined to tell Anna the whole truth about Valerio, but can't find the right moment to address the conversation. Meanwhile, Sara and Lorenzo are invited by Federica, Andrea's wife, to the same bathhouse where Sara and Andrea spent their night out.
Anna, having discovered the truth about her real father, is shocked and decides to lock herself in her room to be alone and reflect. Meanwhile, the Martinis finally decide to build a second bathroom in the house, despite Libero's resistance.
Paolo, Bobò's best friend, reveals to Lele and Edoardo that he is gay and that he is at the mercy of the school bullies. Meanwhile, at the Martini house, Augusto appears, a plumber who immediately shows sympathy for Maddalena.
Anna is increasingly in crisis and has failed the entrance exam. Lele asks Carla to have Anna enroll in the Biology exam which, once passed, will give her access to the Faculty of Medicine.
Anna, on the eve of her exam, is spending the night at a disco. Tommy and Agnese who told their parents they were going to a school film screening, but they are at the abandoned factory.
In order to take revenge on Maddalena, Augusto brings a pestiferous nephew into the house who gets up to all kinds of things. After discovering Augusto's machination, Maddalena scolds him, but he tells her that they are now even and a draw.
Knowing the truth about Sara's betrayal, Lorenzo punches Andrea. Then he gives in to despair and gets drunk. Meanwhile, the financiers of the new surgery department arrive at the clinic, which will be directed by Lorenzo.
Ciccio returns to Rome to see a mare to buy. He is warmly welcomed by the whole family and goes with Lele to Valerio's loft where Geko's record is being presented.
On the plane to Berlin, Sara meets Matteo, a concert organizer who gets her two tickets for a deejay's show. Lorenzo, annoyed by that friendship, tells his wife that something between them is broken forever.
Made aware of the crisis with Lorenzo, Libero goes to the cottage to talk to Sara. The girl shows Libero a nearby grove but just then some gardeners from the Municipality arrive: the trees have to be cut down.
Maddalena, very worried about Rocco's situation, decides to go and look for him. Libero accompanies her, but when they arrive at the address indicated by Maddalena, they find a group of people in front of the door and an ambulance.
Rocco, who has installed himself bag and baggage in the Martini house, does nothing but eat and then lie down on the sofa. When he says he will look for a job, Augusto and Libero follow him and catch him while he is playing video poker.
Lorenzo manages to operate on a little girl with a heart malformation with his own. Celeste and Lorenzo are very happy and compliment each other. They leave the clinic together hand in hand, but Sara catches them in the act.
Lorenzo spent the night with Celeste. Sara decides to participate in the race run by the “Cuore a Cuore” association, but Lorenzo refuses to sign the eligibility form because he considers Sara's competitive activity too risky.
Jacinta goes to Poggiofiorito to meet Rocco and tell him that she intends to get back together with him; Rocco is about to give in when she notices the presence of Libero and Enrica who were spying on them.
Emiliano leaves the Raggio di Sole Center and finds Ginevra lifeless on the ground, after being run over. He calls an ambulance that takes the girl to Villa Aurora, where Lele is informed of the girl's identity.
It's the day of the final of the volleyball tournament. The boys are excited and excited, but realize their coach is distracted and has other things on her mind. Meanwhile Lorenzo receives an embittered message from Sara.
